Physician for Hospice
In order to ensure that the final care plan is in line with the patient's preferences and goals, the hospice physician works in conjunction with the interdisciplinary team. He assesses the patient's health, treats symptoms, writes prescriptions, and suggests tests and treatments that are required.
A registered nurse
The registered nurse manages the care plan, plans physical treatment, keeps an eye on patients to manage symptoms, and sets up prescription drugs, supplies, and medical equipment. He or she also teaches patients and their families how to give the right care and what to anticipate.
A certified nursing assistant
Under the direction of a registered nurse and other medical specialists, certified nursing assistants (CNAs) offer practical support. She provides assistance with movement and exercise, personal care, food, laundry, errand running, and bathing.
Counsellor for Bereavement
A bereavement counsellor helps families and individuals get through the grief process. The goal is to provide them with empathetic counselling so they may examine their feelings, deal with their loss, and discover healthy methods to express it.
A Social Worker
As hospice patients and their families move into end-of-life care, social workers offer them emotional support and counselling. In addition to addressing psychological, social, and resource requirements, they conduct distress assessments.
Chaplain
Chaplains provide hospice patients and their families with nondenominational spiritual and emotional support. If asked, they might also help with funeral planning.
